Dear Assurant Members:

We have enjoyed getting to know many wonderful people through our relationship with Assurant over the years and it is our hope you feel that way as well.

Unfortunately, we find that we can no longer provide for the Assurant HMO Plan. We have worked very hard to make it work for all parties. However, as of January 2012 we will no longer be one of their providers.

Our sincere hope is that you can elect another dental insurance so we can continue our relationship – for those of you who cannot we wish you the best.
